Men with low testosterone have higher rates of insulin resistance, metabolic syndrome, and type 2 diabetes than testosterone-sufficient men. What Did the Largest Cardiovascular Trial Show?
The largest randomized trial on testosterone and heart safety — TRAVERSE, 5,246 men published in the New England Journal of Medicine — found that TRT was noninferior to placebo for major cardiovascular events, with MACE rates of 7.0% versus 7.3% (Lincoff et al., 2023).
TRAVERSE enrolled men with hypogonadism who already had cardiovascular disease or elevated cardiovascular risk — the highest-risk population you would want to study. That population matters: if TRT were going to cause harm, this is where you would have seen it. The noninferiority result — TRT did not increase cardiovascular risk — was the clinically meaningful finding (Lincoff et al., 2023).
This is important framing. Noninferiority is not the same as proven cardiovascular benefit — TRAVERSE was not designed to show TRT prevents heart attacks. It showed TRT did not cause them in men already at elevated risk.

Polycythemia. The flip side: testosterone also carries a risk of driving hemoglobin too high, a condition called polycythemia or erythrocytosis. This is a monitoring issue, not a reason to avoid TRT — but hematocrit checks are a non-negotiable part of responsible TRT management. Most clinics flag hematocrit above 54% as the threshold for dose adjustment or temporary hold (Di Lodovico et al., 2022).
